Google Ads for Skin Clinics: Campaign Structure, Bidding, and Compliance

Graphed Team••Google Ads by industry•9 min read

Most skin clinics burn Google Ads budget on broad dermatology terms and untracked phone calls. The difference between profitable campaigns and expensive experiments: campaign structure that matches treatment intent, conversion tracking that captures offline bookings, and compliance setup that keeps your account active.

Campaign structure that matches treatment intent

Separate search campaigns by treatment tier: entry treatments like consultations and basic facials, mid-tier procedures like chemical peels and microneedling, high-ticket services like laser treatments and injectables. This lets you control budgets and bid strategies based on patient value and conversion rates at each level.

Single Keyword Ad Groups work for your top converting treatments. One keyword per ad group gives you complete control over ad copy and landing page match. When someone searches for microneedling, they see a microneedling ad that lands on a microneedling page. This approach typically delivers better conversion rates than generic dermatology ads.

Exact match on treatment-specific terms keeps you out of expensive informational queries. Treatment plus location searches convert better than broad queries. Broad match "skin treatment" pulls in people researching home remedies and drives lower conversion rates at higher costs per click.

Dynamic Search Ads only make sense if you have 50-plus indexed treatment pages and solid site architecture. Otherwise Google sends traffic to blog posts and about pages instead of booking-optimized landing pages.

Performance Max campaigns work after you have sufficient conversions per month and clean conversion data. Without adequate conversion volume, Performance Max burns budget on display placements and YouTube pre-rolls that deliver zero bookings. Start with standard search until your data justifies automation.

Budget allocation and bid strategy by conversion cycle

Start with adequate daily budget in metro markets. Anything less spreads too thin across multiple campaigns and never generates enough data for optimization. Small budgets mean you spend months learning what a proper budget would have taught you in weeks.

Allocate the majority of budget to branded terms and high-intent treatment searches where people use specific procedure names. The remaining portion goes to discovery terms where searchers describe problems: acne treatment, skin tightening, fine line reduction. Discovery terms convert at lower rates than treatment-specific searches, but they introduce your clinic to patients who haven't decided on a procedure yet.

Run Manual CPC for your first 30 days or until you hit adequate conversions per campaign. Manual bidding prevents Google from experimenting with your budget while you establish baseline performance. After you have conversion data, switch to Target CPA.

Expected cost per click varies widely by treatment type and market. General dermatology terms cost less than cosmetic procedures, which cost less than injectables. Markets with established med spas push the high end. Plan for higher average CPCs across campaigns in competitive areas.

Set Target CPA based on average treatment value, not lifetime value. Google optimizes for the conversion you track, which is the initial consultation or first treatment. Setting it based on lifetime value trains the algorithm on economics it cannot see.

Conversion tracking that captures offline bookings

Phone call tracking matters more for skin clinics than any other conversion type. Most bookings happen by phone. Use Google's call conversion import or integrate CallRail to track which keywords and ads drive calls that turn into appointments. Without call tracking, you optimize half your funnel blind.

Import completed appointments from your booking system back into Google Ads as conversions. Form submissions and scheduled calls are leading indicators, but showed appointments are what matters. Track what actually fills your treatment rooms.

Set up enhanced conversions to match form data with CRM records. When someone fills out a contact form, then calls three days later and books, enhanced conversions connects both touchpoints to the original ad click. Without this, you undercount conversions significantly.

Track consultation shows separately from no-shows. Create two conversion actions: scheduled appointment and completed appointment. Optimize for completed appointments. Some keywords attract tire-kickers who book but never show. Those conversions look good in your dashboard but kill actual ROI.

Exclude internal calls and spam using call duration minimums. Set an appropriate threshold. Very short calls are usually misdials, staff checking voicemail, or spam. Including them tells Google that low-quality traffic converts, so it sends more.

Landing pages that convert treatment searchers

Build dedicated landing pages per treatment with no global navigation. Every exit path costs conversions. If someone clicks your microneedling ad and lands on a page with menu links to your blog, about page, and other treatments, many will click away before they see your booking form. One treatment, one page, one call to action.

Put booking CTA and phone number above the fold in click-to-call format for mobile. Most skin clinic searches happen on mobile. Making someone scroll to find your phone number kills mobile conversion rates. Big phone button at the top, visible without scrolling.

Before and after galleries work only if you have written patient consent and comply with ASA/CAP guidelines in the UK or FTC endorsement rules in the US. Unattributed photos or results described as typical when they are outliers will get your landing page flagged and your ad account suspended. When in doubt, show credentials over photos.

Display starting prices or price ranges. Price transparency pre-qualifies leads and increases conversion rates. People who cannot afford your services click away before they call. People who can afford them feel more comfortable booking because you are upfront about costs.

Show provider credentials and certifications above the fold. Board certifications, medical licenses, years of experience, and professional memberships build trust faster than testimonials. Put credibility signals where people see them immediately.

Compliance requirements that kill unapproved accounts

Healthcare services certification is required in Google Ads for prescription treatments and medical procedures. Submit license verification before you launch campaigns. Waiting until after your ads are disapproved adds weeks while Google reviews documentation.

Ad copy cannot make unsubstantiated claims. "Best dermatologist," "guaranteed results," and specific outcome promises violate healthcare advertising policies. You also cannot reference prescription medications by name in ad text. Describing what you treat and how you treat it is fine. Promising specific results or naming prescription drugs gets ads disapproved immediately.

No before and after images directly in Google Ads creative. Even if before/after photos are allowed on your landing pages under local regulations, they violate Google's ad policies. Image extensions and responsive search ads cannot include treatment results imagery.

HIPAA compliance for remarketing: exclude existing patient lists from ad targeting unless you have explicit marketing consent separate from treatment consent. Serving ads to people based on their patient status is a HIPAA violation. Most clinics should avoid patient list uploads entirely and stick to website visitor remarketing.

Location targeting must match physical service areas. You cannot target cities where you do not have a licensed practitioner providing services. Google checks business licenses and service areas. Overstating coverage gets your account suspended for misrepresentation.

Keyword strategy for bottom-funnel traffic

Treatment plus location combinations as exact match outperform broad "near me" terms on conversion rate. Specific treatment and location searches convert better than broad match "near me" queries and often cost less per click. Exact match keywords take more setup time but deliver dramatically better ROI.

Problem-based keywords like "acne treatment" and "skin tightening" work as phrase match for mid-funnel traffic. These searchers know they have a problem but have not decided on a treatment. They convert at lower rates than treatment-specific terms, but they cost less per click and introduce your clinic to patients early in research. Allocate appropriate keyword budget here.

Negative keywords for DIY alternatives, insurance queries, and career searches save wasted spend. Add negatives for "home kit," "drugstore," "covered by insurance," "dermatology school," "how to become." Review search term reports weekly and add negatives aggressively.

Competitor conquesting only makes sense if you have a clear differentiator and budget to sustain high CPCs. Bidding on competitor clinic names drives expensive clicks from people already committed to another provider. Unless you offer unique treatments they do not, or you have obvious price or location advantages, competitor keywords burn budget.

Exclude "free," "cheap," "reviews," and informational terms. Someone searching "best chemical peel reviews" is far from booking. Someone searching "chemical peel clinic near me" is ready now. Bid on intent, not curiosity.

When Google Ads won't work for skin clinics

Markets with multiple established competitors push CPCs above sustainable customer acquisition cost unless you have brand recognition or unique treatments. New clinics in saturated markets should expect extended periods of unprofitable learning before campaigns stabilize.

Clinics without phone answering during business hours waste significant ad spend on missed calls and voicemails. If your front desk cannot answer incoming calls, or you let calls roll to voicemail after three rings, you pay for leads you never convert. Fix phone answering before you scale ad spend.

No conversion tracking means you optimize for clicks, not bookings. Expect higher CAC without proper measurement because you cannot tell Google which clicks turn into patients. The algorithm sends traffic that generates clicks and calls, but without conversion data it has no idea which traffic actually books.

Landing pages that force multi-step forms or require email before showing pricing kill conversion rates. Streamlined pages with visible phone numbers and single-step contact forms convert much better. Every extra field drops conversion rate.

Running ads without retargeting budget leaves most first-time visitors unconverted. Someone who clicks your ad, reads your landing page, then leaves will forget your clinic name quickly. Plan to spend appropriate budget on display and video remarketing. Search campaigns find new patients. Retargeting campaigns convert them.
